EMPLOYEE IDENTIFICATION FORM CONSENT AND AUTHORIZATION AGREEMENT

This Employee Identification Form Consent and Authorization Agreement (“Agreement”) is entered into by and between Invitation Home Rental (“the Company”) and the undersigned individual (“Employee” or “Applicant”) for the purpose of obtaining, verifying, processing, and maintaining accurate identification and employment-related information. By proceeding to enter, submit, or provide any personal information through this Employee Identification Form, you acknowledge that you have read, understood, and agreed to the terms and conditions set forth below.

Invitation Home Rental is committed to maintaining accurate records of individuals who seek employment with, are employed by, or otherwise provide services to the Company. The information requested in this Employee Identification Form is necessary for legitimate business purposes, including but not limited to verifying identity, confirming eligibility for employment, complying with federal, state, and local laws, administering payroll and benefits, maintaining workplace security, managing internal records, and ensuring adherence to Company policies and regulatory requirements. By consenting to this Agreement, you authorize Invitation Home Rental to collect, store, use, and process the personal information you provide for these lawful and legitimate business purposes.

You understand and agree that the information requested may include personal identifying details such as your full legal name, residential address, mailing address, date of birth, Social Security number or other government-issued identification number, driver’s license or state identification information, contact details, emergency contact information, employment history, and any other information reasonably required to verify your identity and employment eligibility. You further acknowledge that providing accurate and truthful information is a condition of employment consideration or continued employment. Submission of false, misleading, incomplete, or inaccurate information may result in disqualification from employment consideration or disciplinary action, up to and including termination of employment, in accordance with applicable law.

By submitting this form, you expressly consent to the Company verifying the information provided. Such verification may include, where legally permissible and necessary, confirming identity through government-issued documents, validating Social Security numbers, confirming employment history, conducting background checks if separately authorized, and verifying eligibility to work in the United States as required by federal law. You acknowledge that certain verification procedures may be conducted through third-party service providers acting on behalf of Invitation Home Rental. The Company will take reasonable steps to ensure that any third-party service provider engaged for verification purposes maintains appropriate safeguards to protect your personal information.

You understand that Invitation Home Rental will handle your personal information in accordance with applicable privacy and data protection laws. The Company agrees to use reasonable administrative, technical, and physical safeguards designed to protect your personal information against unauthorized access, disclosure, alteration, or destruction. Access to your personal information will be limited to authorized personnel who have a legitimate business need to access such information in order to perform their job duties. While the Company implements reasonable safeguards, you acknowledge that no method of electronic storage or transmission over the internet can be guaranteed to be completely secure, and you accept this inherent risk in providing your information electronically.

Your information may be retained by Invitation Home Rental for as long as necessary to fulfill the purposes outlined in this Agreement, including compliance with legal, tax, regulatory, accounting, and reporting requirements. In the event your employment application is not successful, the Company may retain your information for a period consistent with applicable laws and internal retention policies. If you become employed by the Company, your information will be maintained as part of your employment record for the duration of your employment and for any legally required period thereafter.

By proceeding with this form, you acknowledge that certain information requested may be required by law as a condition of employment eligibility, including documentation required for employment verification and tax withholding documentation. Failure to provide required information may prevent the Company from proceeding with your employment application or may affect your continued employment status if you are already employed. You understand that the Company’s collection of such information is necessary to comply with federal and state legal obligations.

You further consent to the Company using your identification information for internal administrative purposes such as issuing employee identification numbers, security badges, access credentials, payroll processing, benefits enrollment, insurance administration, and other employment-related administrative functions. Where applicable, your information may be shared with government agencies, payroll processors, benefits administrators, insurance carriers, financial institutions, or other service providers strictly for legitimate business or legal compliance purposes. Such disclosures will be limited to information reasonably necessary to fulfill the intended purpose.

If at any time your personal information changes, including your legal name, address, contact information, or work authorization status, you agree to promptly notify Invitation Home Rental and provide updated documentation where necessary. Maintaining accurate and current information is essential to ensure proper payroll processing, tax reporting, benefits administration, and legal compliance. Failure to update your information in a timely manner may result in administrative delays or complications for which the Company cannot be held responsible.

You acknowledge that by selecting “Accept,” signing electronically, or otherwise proceeding beyond this consent page, you are providing your informed and voluntary consent to the collection and processing of your information as described herein. Your electronic acknowledgment shall have the same legal effect as a handwritten signature and shall constitute your agreement to be bound by the terms of this Agreement. If you do not agree with the terms outlined in this consent, you should not proceed with entering your personal information into the Employee Identification Form.

Nothing in this Agreement is intended to alter the at-will nature of employment with Invitation Home Rental where applicable by law. This consent relates solely to the collection, use, and processing of identification and employment-related information and does not constitute a contract of employment or guarantee of continued employment for any specific duration. The Company reserves the right to update or modify its policies and procedures regarding employee information management in accordance with applicable laws and business needs.

By continuing to the Employee Identification Form, you hereby grant your informed consent to Invitation Home Rental to process your personal data as described, and you acknowledge that this consent is given freely, knowingly, and voluntarily.
